Tap Scan to capture paper with the camera, or Import to bring in a PDF or an image from Files or Photos. The app reads the text on your device, then suggests a title, a category, tags, and any important dates it finds.
When the app finds a due date, an expiry, a renewal, or a warranty end, you can save it as a reminder. Reminders are local notifications on your device, so notification permission has to be granted for them to alert you. If you turned notifications down earlier, the Reminders tab will tell you and offer a link to iPhone Settings.

No. On devices that support it, summaries and answers use Apple Intelligence on device. On every other device the app uses built-in on-device rules and works the same way. The summary card tells you which one produced the text.
Not yet. You can export a complete backup package from Settings, which contains your documents and a readable manifest of everything the app knows about them, but the app cannot import one back in. Keep the originals of anything irreplaceable. Restore is the next major feature.
Yes. There is no trash and no undo. Deleting a document also removes its file, its reminders, and any scheduled notifications.
Check that notifications are allowed for the app in iPhone Settings. The Reminders tab shows a banner when they are off. Reminders whose date has already passed are not scheduled.
Settings, then App Lock. It uses Face ID, Touch ID, or your device passcode, and locks whenever the app goes to the background. Your device needs a passcode set for this to be available.
If Hide Widget Content is on in Settings, the widget deliberately shows nothing private. Turn it off to see counts and the next due date.
